Job Description

Community Physiotherapist – Join Us During an Exciting Growth Phase

Your Opportunity

There has never been a more exciting time to join our Community team. As we continue to expand our community services and enter a new phase of growth, we are looking for passionate Physiotherapists who are excited to make a real difference in people’s lives — in their homes and communities, where it matters most.

We are also proud to now be part of a larger healthcare network following our recent acquisition by Australian Unity. This means increased stability, stronger career pathways, and exciting opportunities for growth across the community and healthcare space.

As a Community Physiotherapist, you will join a large, supportive multidisciplinary team delivering personalised, evidence-based care to help consumers maintain independence, function, and quality of life.

This is a part time (4 days/eek) mobile community role in Melbourne's Inner North.

We are open to early career or graduate therapists who are motivated to learn, enjoy variety, and are comfortable travelling between clients.

What You’ll Be Doing

You will manage a varied caseload across acute and chronic conditions affecting movement, function, and independence.

Our Physiotherapists are passionate about delivering high quality care that supports meaningful health and wellbeing outcomes for our consumers.

Duties include (but are not limited to):

  • Delivering comprehensive assessments and clinical recommendations
  • Supporting activities of daily living, mobility, and transfers
  • Falls risk assessment and prevention
  • Equipment prescription and recommendations
  • Pressure area care and skin integrity management
  • Arthritic joint and pain management
  • Working collaboratively within the multidisciplinary team
  • Adapting to the needs of community-based service delivery

Our community clients are referred through:

  • Home Care Packages
  • Short Term Restorative Care
  • Commonwealth Home Support Programme
  • Private services

We are not your typical community provider — we set achievable, realistic billable KPIs (ask us more!).

Your Career With Us

In addition to a genuinely supportive team environment and competitive salary (reviewed annually), we pride ourselves on real work/life balance and flexibility.

You’ll be supported with structured clinical pathways that combine:

  • Consumer goals
  • Clinical presentation
  • Latest evidence-based practice

All delivered in the consumer’s home environment to achieve meaningful outcomes.

Our Perks

When you join our community team, you unlock a range of benefits including:

  • Industry-leading Professional Development program (≈ $5,000 annually)
  • PD allowance you control
  • Monthly profession-specific training
  • Webinars and Lunch & Learn sessions
  • Paid induction and training
  • Social club and team connection initiatives
  • Health Kick allowance for fitness and wellbeing
  • Recognition programs
  • Ongoing mentoring and clinical support
  • Treasure Chest Fund — funds to brighten a client’s day
  • $1,000 Refer-a-Friend bonus

Welcome to Plena Healthcare

We deliver high-quality and personalised mobile allied health services to Australians in residential aged care facilities and community settings.

Our name ‘Plena’ reflects our ethos– it comes from Latin and means ‘holistic’– and this drives everything we do.

At Plena Healthcare, we offer more than allied health services– we offer care, connection and community. Our passion for what we do enables us to create strong relationships with clients, staff and families, so we can provide optimal healthcare for all ages and life stages.

Our services include:

• Physiotherapy

• Occupational Therapy

• Podiatry

• Speech pathology

• Dietetic services

We’re here to make life easier for you. Because of our history and experience, we understand how to integrate with aged care facility teams, offer cost-effective clinical excellence, and holistic healthcare solutions– all delivered with a smile.

We also love what we do, and who we do it with. That makes a huge difference, as we genuinely love working in aged care and with residents, and making their day.

Plena Healthcare is the largest provider of allied mobile health services in Australia. Every day, we work together united by a common purpose: to provide quality care so people can live healthier, happier and more independent lives.

We’re operating in a space that is growing. The demand for healthcare including physiotherapy, occupational therapy, podiatry, dietetics and speech pathology is rapidly growing and to meet this demand, we’re in the process of expanding our team to help more customers, residents and communities flourish.

Collectively, we’ve been around for more than 20yrs as a number of trusted and well-established brands. You may have known us previously as Xtra AgedCare, Transform Physio, Dimple, Loqui Speech Pathology, Leading Nutrition Dietetics or Allied Care Group.

But now we are connected One and All as Plena Healthcare
