Job Description

Compensation Range: $18/hr - $19/hr commensurate with experience

Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.

This position supports Housing Works mission to connect new and existing clients/patients to programming through an efficient and friendly customer experience. The Community Liaison works as an Extender to the Account Management team and provides a warm welcome for the clients the Account Managers are connecting to Housing Works. The ideal candidate is outgoing, personable, articulate, organized and persistent in their mission to connect individuals to care.The Community Liaison ensures that clients/patients have an unparalleled, excellent experience at Housing Works Community Health Center. The person in this position would have to be sensitive, flexible and responsive to gender, race, ethnicity, socio-economic status, religion, age, sexual orientation or other special needs as reflected in the ability to communicate with staff and/or clients. They would also have to uphold and advance the values, ethics, knowledge and mission of Housing Works.

Responsibilities:

  • Maintain knowledge of Housing Works' programming, screening tactics and eligibility requirements as well as effectively articulate the benefits to eligible clients/patients
  • Assist in enrolling clients or registering patients in programming and helps clients access entitlements, health insurance and benefits if barriers present
  • Coordinate client transportation and ensure client adherence to initial visits and appointments
  • Navigate new client/patients throughout Health Center site during day of appointment and deliver the patients’ courtesy lunches during their visit
  • Provide follow-up support to ensure client/patient adherence to referrals made including follow-up calls and/or emails to Partner/Case Manager
  • Document and critically think through possible solutions for customer service issues
  • Assist team in organizing outreach efforts, promotional materials and producing data reports that help improve quality of partnership
  • Meet documentation and reporting requirements on a weekly and monthly basis to track client engagement and enrollments
  • Participate in professional training and development opportunities including certifications that may be required
  • Active member of the Community Partnership team participating in weekly huddles, monthly meetings and participating in community events as needed

Requirements:

  • High School Degree, associate’s degree, bachelor’s degree preferred or in pursuit of
  • One-two year experience working in community-based setting including health services as a client/patient navigator, outreach worker or other relevant experience
  • Demonstrated customer service skills including problem-solving
  • Organized and able to handle multiple tasks and work in a fast-paced environment
  • Able to work independently while also supporting the team
  • Excellent verbal and oral communication skills that are timely and effective including phone etiquette and concise emails
  • Exceptional interpersonal skills; able to conduct him/herself in a diplomatic and professional manner
  • Accurate, precise and timely in reporting and managing numerous admin task
