Ute Pass Regional Health Service District is looking for compassionate, clinically strong paramedics who want to help shape the future of mobile integrated healthcare. As a Community Paramedic, you will meet patients where they are, provide thoughtful in-home and community-based care, support mental health and resource connection, and help ensure the right level of care is delivered at the right time.

The Community Paramedic serves as both a cleared Paramedic and an expanded community-based clinician within UPRHSD’s Mobile Integrated Health program. This position responds across the District’s service area, supports patients who may not need emergency department transport, and helps connect individuals with appropriate care, resources, follow-up, and support.
  • Respond to a wide range of calls, including community-based assessments, mental health situations, non-emergent interventions, and traditional 911 ambulance assignments when needed.
  • Provide advanced clinical assessment, point-of-care services, medication support, care coordination, telehealth connection, and alternative destination support within approved protocols and scope.
  • Work collaboratively with the Medical Director, Deputy Chief of Clinical Services, operations leadership, partnered providers, and community resources.
  • Support patients experiencing complex health, behavioral health, or social needs by helping them navigate care options and follow-up support.
  • Contribute to the continued growth of the MIH program through quality improvement, case review, community outreach, training, and professional collaboration.

Qualifications

Required
  • Current Colorado Paramedic certification (within 30 days of hire)
  • Current AHA Basic Life Support (BLS) certification
  • Current AHA Advanced Cardiac Life Support (ACLS) certification
  • Current National Registry of Emergency Medical Technicians (NREMT) Paramedic certification
  • Valid Colorado driver’s license with an acceptable driving record
  • Proof of current private automobile insurance coverage
  • Proof of completed HBV series, TB test, and other immunizations as requested by the District
  • Must provide copies of all new or renewed required certifications and licenses to the District
Preferred
  • Prior experience in community paramedicine, mobile integrated health, or a community health role
  • Mental health first aid, crisis intervention training, or behavioral health-specific certifications
  • Motivational Interviewing (MI) training or certification
  • Experience with Medication Assisted Treatment (MAT) programs or substance use disorder support
  • Experience working with high-utilizer patient populations, underserved communities, or complex social determinants of health
  • Familiarity with local community resources, social service agencies, and healthcare referral partners in the District service area
  • Experience with telehealth platforms or remote patient monitoring tools

Additional Information
  • Grant-Funded Position This is a grant-funded, limited-term position anticipated to continue for up to five (5) years. Continued employment is contingent upon the availability of grant funding and ongoing program and operational needs. The position may end earlier if grant funding is reduced, discontinued, or otherwise becomes unavailable.
  • Schedule Assigned shift rotation as determined by the District; current schedule consists of 12-hour shifts on a rotating schedule of three (3) days on/four (4) days off, followed by four (4) days on/three (3) days off. Schedules are subject to change based on operational needs, system metrics, and District requirements.
  • Role Structure Community Paramedics are expected to function as proficient paramedics first, then apply expanded Community Paramedic training and judgment within the MIH program.
  • Training Candidates will be required to complete 911 ambulance field training and Community Paramedic field training in accordance with District standards.
  • Program Focus This role supports community-based care, mental health support, care coordination, telehealth connection, alternative destination pathways, and responsible use of EMS resources.
