STATEMENT OF THE JOB
The Community Outreach Specialist works in collaboration with staff, volunteers, and interns in the execution of assigned tasks including but not exclusive of service coordination, comprised of intake screening, needs assessment, information and referral, community linkage, and outcomes follow up. This work is conducted in a variety of settings: satellite locations and community events throughout Brooklyn & Queens. Responsibilities include but are not limited to: adherence to all administrative and data collection policies, collaboration with internal and external networks, and engagement in community building activities in support of the agency’s mission. This work will be completed within the model of care which is holistic, family centered, strengths-based, and serves to empower clients and enrich the community.
The agency has embraced the concept of a fully integrated system of care for all consumers of service. This requires that the staff assess the client's needs holistically and see it as their responsibility to connect the client with services needed--internal or external to the agency, regardless of the door to which the consumer enters the system.

For over 125 years, Catholic Charities Brooklyn and Queens has been providing quality social services to the neighborhoods of Brooklyn and Queens, and currently offers 160-plus programs and services for children, youth, adults, older adults, and those struggling with mental illness. As one of the largest faith-based developers of affordable housing in the country, Catholic Charities provides more than 4,500 units of housing for low-income older adults, families, and the formerly homeless, throughout Brooklyn and Queens. With the country reeling from an economic crisis, countless members of our community in Brooklyn and Queens have turned to Catholic Charities for assistance. In response, we increased our efforts to address issues of poverty and promote justice in the communities we serve, thereby empowering families and individuals to achieve greater self-sufficiency.
