One Health's Central Montana Health District team is looking for a Community Outreach Coordinator to join our team in Lewistown, MT.

Location: Home base is Lewistown, MT however, this position is part of the Central Montana Health District sector of which serves 5 surrounding counties.
Schedule:Full-time |Flexible Hours|Travel Required
Pay:$20.55 - $21.37/hr DOE

One Health is seeking a Community Outreach Coordinatorto promote health, connect individuals to resources, and strengthen rural communities through education and outreach. If you’re passionate about helping others and thrive in a collaborative environment, we want you on our team!

What You’ll Do:

• Conductoutreach to promote One Health services and programs.
• Assistindividuals in developing health/social management plans and goals.
• Designand implementprevention campaigns and education programs.
• Documentencounters and maintainaccurate electronic records.
• Buildrelationships with healthcare providers, agencies, and tribal entities.
• Participatein community events, meetings, and trainings.

• Ability to manage multiple priorities independently
• Professionalism, empathy, and clear boundaries
  

Required:

• Education:Associate’s degree OR 4 years’ related experience
• Experience: One (1) year of experience in Community outreach. Demonstrates advanced knowledge in community outreach to complete essential duties.
• License:Valid Driver’s License
  
• Certification: BLS (Upon Hire)

Preferred:

• Bachelor’s degree in health promotion, public health, health administration, or related field
• Certified Application Counselor (CAC) for Affordable Care Act (ACA)
