
Galloway Ridge is an upscale continuing care retirement community offering top-quality care in a warm, welcoming environment—and we’re looking for a compassionate, detail-oriented Community Navigator to join the Arbor team. The Arbor Community Navigator provides psychosocial support to residents and families adjusting to the Arbor, is responsible for managing changes in health status and serves as a liaison between the resident and community agencies. Responsibilities include collecting information in order to formulate resident social history, psychosocial assessment, formulation of resident care plan, completing assigned sections of the MDS, participation in interdisciplinary team meetings, maintaining up to date progress notes, communicating with internal and external health care providers, crisis intervention, assist in in transitions of care, and any other duties as assigned by supervisor.
