ABOUT CUIDADO CASERO HOME HEALTH AND HOSPICE

The Community Marketing Liaison is responsible for interaction in the community and promoting company services and other duties as deemed necessary.
1. Develop agency census by working with local and regional physicians, medical groups, medical facilities and the community to establish referrals.
2. Increase referral base by at least 15% each quarter.
3. Identify decision-makers at various referral sites.
4. Educate physicians, physician and hospital staff, and discharge planners about the services available through home health and Hospice.
5. Assist with seamless completion and delivery of all paperwork necessary to help facilitate timely patient care.
6. Assist agency in maintaining compliance by delivering physician orders to offices and facilitating getting signature.
7. Discuss care plans with physician offices for patient issues which need to be addressed.
8. Aid in issues resolution related to patient care and referral source satisfaction.
9. Establish self as a loyal and trustworthy liaison and patient advocate with clinicians, families and patients.
10. Maintain and provide to Agency and Corporate accurate business activity/outreach reports, follow up and results.
11. Write and contribute to marketing plans and execution.
12. Prepare and deliver marketing and educational materials, as well as presentations on the home health and Hospice industry and company product lines and services.
13. Stay current with regulatory changes in the home health industry.
14. Maintains contact with referral sources to update them on changes in Medicare/Medicaid regulations.
15. Maintain committed and cooperative attitude with all job duties.
16. Be available to assist the community with wellness and prevention.
17. Assist with seamless completion and delivery of all paperwork necessary to help facilitate timely patient care.
18. Prepare and deliver marketing and educational materials, as well as presentations on the Hospice/Home Health industry and company product lines and services.
19. Stay current with regulatory changes in the Hospice/Home Health industry.
20. No person will knowingly offer to pay or agree to accept any remuneration directly or indirectly in cash or in kind, to or from any person, firm, associate of persons, partnerships, or cooperation for securing patients.
21. No person shall advertise false, misleading, or deceptive information that is not readily subject to verification.
22. Perform any and all other duties and responsibilities as assigned by your supervisor.
COMMUNITY EDUCATION PLAN:
1. Implement community education plan staying within established timetable.
2. Review and evaluate the analyses and plan on an established basis.
3. Stay current with regulatory changes in the home health industry.
4. Prepare educational materials.
5. Know your company and the product lines.
6. Create a plan on contact calls every 2 weeks to be reviewed with DOS.

Qualifications1. Bachelor’s Degree preferred.
2. One year of experience in physician and community relations, and proven background in home healthcare/hospice or related field required.
3. Preferred background with a Medicare Qualified facility.
4. Must be well organized with ability to document all business activity, follow up and results.
5. Experience in working with professional and clinical personnel and community representatives.
6. Must possess effective oral and written communication skills, excellent interpersonal skills and problem solving skills.
7. Respect for human dignity and patients’ rights.
8. Must possess basic computer skills necessary to utilize the internet, and all programs in Microsoft Office Suite with training, skills necessary to utilize other data-driven software programs will be required.
9. Good communication skills and ability to establish good rapport with other staff members.
10. Must practice a high level of confidentiality and be able to effectively response to questions from employees, managers and outside entities
11. Must have knowledge of all company policies and procedures. Must be able to work on a timetable, follow instructions, responds to management direction and solicits feedback to improve performance. Should have excellent character references and solid work background.
12. Must be able to read, write and speak English.
13. Must have knowledge and proficiency of office computer equipment. Demonstrate ability to multi task and work in a fast-paced office setting. Proven ability to cope with conflict, stress and crisis situations.
DRIVING & OTHER REQUIREMENTS
Must have and maintain a valid state driver license, have a registered and reliable vehicle that meets state law standards, and meets the insurance standards of Company. Position requires travel between company sites as well as the community. Traveling by car or airplanes to local or out-of-town meetings, seminars, and conferences.
