Job Title: Community Health Worker (Community-Based Case Manager)
Department: Outreach and Navigation
Reports To: Outreach and Navigation Manager
FLSA Status: Non-Exempt

Position Summary
NOHN is seeking a compassionate Community Health Worker (CHW) to serve as a Community-Based Case Manager. This role provides individualized case management addressing Social Drivers of Health (SDOH) barriers, and includes community outreach, event participation, and partnership-building. CHWs engage with diverse communities to build trust, advocate for patients and systemic improvements, and serve as a bridge between NOHN, community partners, and the populations served.
The ideal candidate is open-minded, adaptable, emotionally resilient, and passionate about improving individual and community health. Spanish and/or Mam skills are strongly preferred.

Essential Duties and Responsibilities
Community Based Case Management
- Manage a panel of patients, conducting assessments to identify SDOH barriers.
- Develop, implement, and monitor individualized care plans using SMART goals.
- Connect patients with community resources to reduce health and wellness barriers.
- Ongoing support using trauma-informed and motivational interviewing approaches.
- Document encounters and progress accurately in the Epic EHR and other software.
- Advocate for patient needs and communicate relevant information to care teams.
Community Outreach and Engagement
- Represent NOHN at community events, health fairs, and convenings.
- Conduct and support outreach and tabling at partner agencies, and community sites.
- Build and maintain relationships with community partners and organizations.
- Serve as a liaison between community members, NOHN and other health systems.
- Promote available services and facilitate connections to care.
- Other duties as assigned.

Qualifications

Required Qualifications
- Valid Washington State driver’s license, clean driving record, and reliable vehicle.
- Basic computer proficiency (web navigation, Microsoft Office Suite).
- Strong organizational and time management skills.
- Ability to work independently and within a team.
- Willingness to work in community-based settings.
- Commitment to serving diverse populations with openness and respect.
Preferred

Qualifications
- High school diploma, GED (college coursework or degree a plus).
- Spanish and/or Mam ability.
- Experience working with non-English speaking communities.
- Experience in case management, outreach, or social services.
- Knowledge of SDOH and local resources systems.
- Experience with EPIC EHR.

Work Environment
Attend community events, occasional home visit to clients, fast-paced community health center setting, potential exposure to infectious diseases, interaction with diverse patient population.
Benefits
