The Community Health Worker (CHW) promotes health and wellness by building trust, advocating for members, and ensuring timely, appropriate care. CHWs provide outreach, education, care coordination, and navigation support after facility discharge, working closely with the Transition Care Nurse to develop and implement transition plans. Responsibilities include assisting with follow-up appointments, educating patients and families, and communicating changes in condition to the nurse for proactive interventions. CHWs address health barriers, encourage treatment adherence, connect members to health and social services, and reinforce self-care plans. They deliver culturally sensitive education, conduct health risk screenings, and collaborate with healthcare and community professionals to improve outcomes, satisfaction, and overall well-being.
