Position Title: Community Health Worker / Promotoras, Manager

Status: Full Time

Department: Policy & Community Relations Department

Position Reports to: Director of Public Policy & Community Organizing

Purpose of Position: The Community Health Manager will be responsible for assisting patients, their families, and the community to navigate and access community services and adopt healthy behaviors; manage grant deliverables, work cohesively with the Policy, Community Organizing and Quality Improvement departments to achieve an integral and coordinated quality of care. In addition, will work to identify, recruit, train and sustain the nationally recognized Promotora model of Clinica Romero.

Areas of Responsibility:

• Responsible for establishing and maintain trusting relationships with patients and their families by providing general support and navigation within clinic and community.
• Establish and develop relationships with the residents and organizations in the community by conducting outreach and participating in external events.
• Identify, recruit and retain community health workers (promotoras).
• Responsible for the management of the Community Organizing Department including program planning, enforcing of policies and procedures, CHW supervision, budget management and consistent evaluation of departmental goals and objectives.
• Execute grant ongoing deliverables and reporting; maintain clear and concise files.
• Implement outreach and education strategies
• Maintain monthly calendar of health related events for outreach
• Educate and become aware of each departmental program.
• Advocate for the needs of the communities by connecting members to available resources and advocating for basic needs. Conducting policy advocacy for communities and its members.
• Develop and implement specific campaigns aimed at reducing and preventing the social and health problems in the community, in order to move residents towards greater health equity.
• Collaborate closely with pertinent agencies and local government offices including, Mayor’s office, CHIRLA, and CARECEN, to offer and promote available immigrant services.
• Attend weekly operations meetings
• Serve as a liaison with the CA Endowment and USC Good Neighbors
• Organize and conduct neighborhood meetings, forums, and other community activities.
• Assist in the implementation and training of skills training sessions with community residents on topics such as environmental approaches to disease prevention, group leadership, effective communication and listening skills, project planning and evaluation and community organizing.
• Provide appropriate referrals to prevention related services to community residents.

Qualifications/Requirements:

• Excellent verbal, written communication and interpersonal skills
• Bachelor’s degree in Social Services, Public Health or related field; or a minimum of 5 years of experience in program administration and supervision.
• Bilingual English and Spanish.
• Ability to communicate with a variety of audiences such as government program monitors, community agencies’ staff members, and members of the target community.
• Good networking skills for purpose of arranging and coordinating presentations.
• Strong computer skills and the ability to navigate web-based systems, including Microsoft office.
• Requires a flexible work schedule, including evenings and weekends.
• Valid California driver license and be insurable for car insurance.
• CPR certification.
