The Community Health Worker assesses patient needs and assists with providing access and navigation to inpatient, community-based, and/or clinic-based services to those identified as being at-risk of serious health conditions or high utilization of the Emergency Department based on social determinants of health.
The Community Health Worker
Education: High School diploma or equivalent required.
Experience: 2 years of general work experience.
Licensure, Certification, and/or Registration: none. Life Support: AHA BLS-HCP must be obtained and maintained.
Other Minimum Qualifications: Excellent interpersonal and communication skills. Must have reliable transportation, a valid driver’s license, and proof of insurance. Computer word-processing and data entry skills required. Strong collaboration skills. Must work well with other staff, community, and family members. Extremely reliable, dependable, punctual, and honest.

Compensation Range:
$18.70 - $26.18
Employee pay is based on a variety of factors, including qualifications, credentials, education, and prior work experience. The compensation range does not include benefits or pension plan.
