
Under the supervision of the Community Health Worker Coordinator of Ibero-American Action League, in Rochester and overseen by the Director of Community Health Worker Apprenticeship Programs, the Community Health Worker Navigator-WNY serves as a key point of contact for individuals seeking assistance from Ibero Niagara Falls. This role involves screening, assessing, and providing referrals to Medicaid-eligible clients who may need assistance with housing, food, health care, or other essential human services. This role will actively provide appropriate community agency referrals as well as perform various clerical duties. Qualified candidates are preferably bilingual but not required.

This position holds a relatively high degree of independence within the framework of the agency’s policies and procedures. Must exercise considerable autonomy and initiative and make decisions regarding completion of tasks and monitoring activities. Must use sound judgment in reaching those decisions and use established policies and procedures in the day-to-day direct service tasks.
Physical Demands:
The position does require occasional standing, squatting, and lifting to approximately 40 lbs. And frequent sitting.
