This is a dual role as Community Health Work (CHW) is embedded into all our diabetes prevention and management work.
As a Community Health Worker (CHW), you will promote health in the community and with individual members in a culturally competent and responsive way. You will act as a liaison and advocate to improve relationships between historically marginalized groups, service institutions, and the YMCA of WNC through education and referrals. You will connect community members to services specific to their needs, act as a liaison with community organizations, and assist in creating community outreach and health-related education activities. You will provide support to the Food Access Team of the YMCA of WNC in various ways including mobile markets, processing food at the Nutrition Hub, etc.
As a Certified National Diabetes Prevention Program (NDPP) Lifestyle Coach, you will screen and educate individuals at outreach events, facilitate group sessions using the CDC-approved curriculum, drive behavior change per the goals of the NDPP, provide accountability by tracking and reviewing class documents and participant logs, foster group support by creating a safe and encouraging environment that builds peer support and collaborative problem-solving to overcome lifestyle barriers, maintain program fidelity by delivering high-quality group facilitation and coaching and maintain excellent record keeping in compliance with HIPAA regulations.
