Job Description

Community Health Worker in Social Services

Job responsibilities:

  • Work collaboratively with care team of doctors, nurses, and social workers, among other specialties.
  • Coordinate care for patients and families, including medical and social needs.
  • Complete face to face visits, evaluate care plan progress; communicate results/coordinate next steps with the care manager.
  • Monitor ongoing needs of the patients/families to remove barriers, ensure efficiency and help improve access to care and resources.
  • Schedule appointments for members/families.
  • Complete required documentation linked to the care plan.
  • Provide updates to families regarding patient condition and progress; refer families to appropriate resources for information to answer questions and address needs.
  • Consult with care team members to address complex issues.
  • Supports population outreach to identify population(s) at risk and assist with closing population gaps.
  • Location Burnet Campus, 3333 Burnet Ave., Cincinnati OH 45229 (Please note this job does not offer remote work options.)
  • Shift/Schedule Day Shift/40 hours per week, Monday-Friday 8a-4:30p.
  • Schedule hours can be flexible if we meet patient needs.
  • No evenings, weekends or holidays; no on-call.
  • Pay starts at $20.00/hour and can go up based on years of relevant experience.

Required qualifications:

  • Candidates must have a high school diploma or equivalent,
  • 2-5 years of work experience in a related job discipline
  • Have a reliable vehicle/valid driver's license.

  • Current Community Health Worker (CHW) certification in the state of Ohio is preferred but not required.
