Pay Range: $47,500 - $88,000

The Community Health Worker (CHW) serves a critical role in advancing health and wellness within the community by fostering trust, advocacy for individuals and families, and facilitating timely access to appropriate care. The role performs outreach, offer educational resources, coordinate care, supporting closing care gaps, and connect members with relevant community services to address social determinants of health (SDOH). Collaborates with healthcare and community professionals to optimize health outcomes, member satisfaction, and overall community well-being. Participates as a member of the interdisciplinary care team (ICT).
