The CHW – Breast & Cervical Cancer Program (BCCP) will primarily be assisting patients with getting breast and cervical cancer. The position will assess if patients meet the criteria for the breast and cervical cancer program (BCCP) and assist with helping them apply for the program when appropriate. The CHW will assist eligible patients with their social determinants of the health needs by connecting them to resources. The position will assist patients through a variety of methods, including clinic visits, phone visits, and home visits. CHW's will work closely with medical providers, staff, and other agencies to improve patient care and outcomes.

Requirements:
• Any combination of 3 years health/social services experience and/or education
• Verifiable good driving record and reliable transportation
• Background check
• Bilingual (Spanish/Haitian Creole/Somali/Nepali) encouraged to apply
