1008370 CHNA - EH SSCommunity Health Workers (CHWs) are natural helpers and individuals who have a close understanding of the communities they serve. They are specifically hired and trained to address behavioral and social determinants of health through patient-centered goal setting. CHWs reach beyond clinic walls to help patients meet their goals. They creatively leverage patient, family, and community resources to address root causes of patient’s health concerns. CHWs work to increase access to medical, behavioral health, and community services. The goal of this team is to improve the patient’s overall wellbeing through developing trusted relationships with the patient and their family.
