Under the direction of the Greenup County Health Department (GCHD), the Community Health Worker (CHW) serves as a frontline public health professional and trusted liaison between the health department and the communities it serves. The CHW primarily supports the Public Health Dental Hygiene (PHDH) Program by promoting oral health, coordinating patient care, conducting outreach activities, and addressing barriers to care among underserved populations. The position may also provide support to other public health programs, including Overdose Prevention services, immunizations, family planning, WIC, and community health initiatives.
The Community Health Worker works collaboratively with schools, Head Start centers, long-term care facilities, community organizations, and healthcare providers to improve access to services and connect individuals with resources that promote health and well-being.If you have any questions or would like any more information about this job posting please contact Jackie Applegate at jacklynd.applegate@ky.gov.

Typical Working Conditions and Physical Requirements
Work is performed primarily in clinics, schools, long-term care facilities, community settings, and office environments. Frequent travel throughout FIVCO ADD Counties is required. Evening and weekend work may be necessary for outreach activities and special events.
The position requires standing, walking, bending, lifting, and transporting materials and supplies weighing up to 30 pounds. Interaction with patients and community members is constant and may involve working with individuals experiencing illness, substance use disorders, or social and economic hardships. The ability to work under stressful conditions and adapt to changing priorities is essential.
Annual Increment and Benefits
There will be a Six (6) month probationary period. A 5% salary increment will be considered based on the performance of a passing review after the completion of the probation period.
The standard work week will consist of 37.50 hours per week. This employment is designated as Full-Time employment consisting of 26 bi-weekly pay periods, (1,950 hours per year). Benefits will be provided as defined for the Full Time Employee by the Department for Public Health Personnel Regulations 902 KAR Chapter 8 to include Retirement, Vacation, Sick, Health Insurance, Dental Insurance, Vision Insurance and Life Insurance. The employee will be obligated to contribute a share for any of these benefits as defined by the Department for Public Health, Kentucky Retirement System, Kentucky Employer’s Health Plan, or any other governing entity. Other benefits granted by the Greenup County Board of Health may be offered at the discretion of the Director/Appointing Authority. Holidays will include the same holidays made available to merit system employees which are currently:

Starting Salary $17.50 - $20.00/hr. Negotiable with experience.
