COMMUNITY HEALTH WORKER PRN
Purpose
Works to promote, maintain, and improve health and well-being through informal support and counselingto their patientsUsesculturally appropriate outreach,support, andnavigationto helppatientsaccesshealthcareandaddress identified social needs Playsa key roleinbuilding individual capacity by increasingpatient’shealth knowledge and self-sufficiency through a range ofactivitiessuch ashealtheducation,goal setting,motivational interviewing, and advocacy.Workscollaboratively with otherhealthcare team and community-basedprovidersto improvepatienthealth outcomes andreduceracial, ethnic, and economic disparities in those outcomes.

Process Focus:Utilizes multiple system applications forcaseload management,data collection, and reportingUsesElectronic Health record and embedded toolsfor care coordinationand documentationUsesapplicable resources, services, and processes to complete relevant referrals andapplications inatimely manner.Collaborates on performance improvement activities as indicated by program outcomes and patient experience.Incorporates basic knowledge ofTrinity Healthpolicies, practices & processes to ensure quality, confidentiality & safety are prioritized. Demonstrates knowledge of departmental processes & procedures & ability to readily acquire new knowledge.
Communication:Employs effective & respectful written, verbal & nonverbal communications; Develops an environment of mutual confidence & trust through collaborative relationships; Effectively communicates goals, standards, program expectations, service performance & how the work serves Trinity Health objectives Serves as a liaison between providers and patients to improve communication and coordination of services.Provides support to patientsviaphone andin-person interactionsatpatient’s home, community, orclinic setting.

Minimum Qualifications
High school diploma or GED
Completes CHW training/certification within 6 months of employment.
At leastone (1)year of communityinvolvement experience, including volunteer experience
Must complete CHW training/certification within 6 months of employment.
Demonstrated lived experience(such as experiencing poverty, food insecurity, housing instability, substance use, etc.)and/or culturalbackgroundof thepatientcommunity in which this position will work in and/or with.
