The CalAIM Community Health Worker (CHW) serves as a vital link between healthcare systems and vulnerable populations under California's CalAIM (California Advancing and Innovating Medi-Cal) initiative. This role focuses on addressing social determinants of health, improving health outcomes, and reducing health disparities in underserved communities.
Key Responsibilities:
- Conduct community outreach to identify and engage with high-risk, hard-to-reach individuals.
- Perform health screenings and assessments to determine client needs.
- Provide health education and promote preventive care practices.
- Connect clients with appropriate medical, behavioral health, and social services.
- Assist clients in navigating complex healthcare and social service systems.
- Advocate for clients' needs within healthcare and community settings.
- Maintain accurate and timely documentation of client interactions and outcomes.
- Collaborate with interdisciplinary care teams to ensure comprehensive care delivery.
- Participate in ongoing training and professional development activities.

Schedule:
- 8 hour shift
- Evenings as needed

Qualifications
- High school diploma or equivalent; associate or bachelor's degree in a related field preferred.
- Certification as a Community Health Worker or willingness to obtain certification within 18 months of hire.
- Minimum of 2 years of experience in community health, social services, or related field.
- Strong understanding of local community resources and social determinants of health.
- Excellent interpersonal and communication skills.
- Proficiency in electronic health records and basic computer applications.
- Bilingual skills (English/Spanish, or other relevant languages) highly desirable.
- Valid driver’s license and reliable transportation.
