Responsibilities

Job Summary

Perform direct outreach to target population as listed on program grant and/or as directed by supervisor.

Essential Job Functions

  • Provide basic information on health topics as directed by Supervisor• Conduct comprehensive outreach (and/or in-reach) in local area, and refer individuals to program grant activities and/or clinic• Complete appropriate paperwork• Conduct basic assessments/screenings of target population• Complete data collection forms• Participate in health fairs and other community events• Track incentive cards• Demonstrate familiarity with VCC programs and services• Meet minimal requirements of program objectives• Database entry• Attend meetings, as needed• Enhance professional growth and development through participation in educational programs, current literature review, in-service meetings and workshops• Support the vision, mission and goals, and demonstrate a commitment to the values, of the organization• Perform other duties as directed

Qualifications

Minimum

  • High school graduate or equivalent
  • Minimum of two years’ experience
  • Bilingual English/Spanish
  • Valid California driver’s license and vehicle insurance; reliable transportation, to travel for meetings and/or to gather information for reporting purposes; minimum two years’ driving experience; no more than 2 violations or a single ticket/accident value at more than 1 point on driving record

Required Skills/Knowledge/Abilities

  • Demonstrated ability to communicate effectively orally with target population and VCC staff
  • Effective verbal and written skills necessary for communication with patients/clients, clinicians and other staff
  • Ability to interface with all levels of personnel in a professional manner, including people of all social, cultural and ethnic backgrounds and within the constraints of government funded programs
  • Ability to work independently, take initiative, and be proactive
  • Ability to track and manage time effectively
  • Ability to handle confidential materials and information in a professional manner
  • Ability and willingness to provide outreach throughout targeted areas including on the street, door-to-door, in retail outlets, businesses and other venues as needed
  • Ability and willingness to meet the organization’s attendance and dress code policies

Pay Range

  • $21.00 - $23.00 Depending On Experience
