Hennepin Health is seeking two Special Needs Basic Care (SNBC) Community Health Specialists to join the SNBC team to provide care navigation for members who are certified disabled focusing on the needs of members with mild-moderate complexity. These positions will engage directly with members to offer care navigation and support including establishing primary care, addressing systemic barriers, and facilitating other supportive resources.
These SNBC Community Health Specialists will work within an interdisciplinary team of nurses, other community health specialists, and a social worker to sensitively address the social determinants of health of SNBC members. These roles will focus on a vulnerable population and will work to address their needs in a trauma-informed and member-centric way. Individuals filling this role must be flexible, innovative, and understanding of the challenges and unique health conditions experienced within the SNBC population.
These positions are hybrid and will be performed both on-site at the Health Services Building, 525 Portland Avenue South, 8th floor, Minneapolis, MN 55415, in the field and community-based settings, and remote as job duties require. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. While these positions are designated as hybrid, based on current requirements, hires must reside in or relocate to Minnesota or Wisconsin.
These are full-time, benefit earning positions. You must be a current regular or probationary employee of Hennepin County to be eligible for these positions. Note: Limited duration employees are not eligible.

In these positions, you will:Provide appropriate follow-up to members while conducting this work in the field, community settings, and over the phone to accommodate member needs. Regularly contact members through various means to ensure needs are being met.Manage a caseload of SNBC members with varied disabilities and needs by providing responsive follow up and management of individualized care plans, facilitating voluntary check-ins with members, and engaging with members who may not wish to have a Health Risk Assessment completed.Work within MnChoices and Health's Edge Guiding Care, a care management system, to create and submit assessments, support plans, track member encounters, manage DHS required due dates, manage activities and actions, and maintain an accurate repository of case information for each member in the care navigator's caseload. Prepare and maintain records including operational, regulatory, and utilization reports while adhering to and achieving compliance with all contractual requirements specified by the Minnesota Department of Human Services (DHS). Participate in team meetings, internal and external case review, member events, and establish and maintain effective relationships with other case management staff to effectively coordinate member care.Need to have:One of the following:Bachelor's degree or higher in public health, nursing, behavioral/social science, or an approved related field.Four years or more of related professional experience providing direct service to assess and support client needs, organizing and reporting on programmatic data, health education, counseling, outreach, and referral services with the community. Experience:Completing internal and external administrative requirements for large caseloads using multiple systems while managing the daily actions of member engagement, researching resources, and identifying community partners to help provide SNBC members with services.Meeting members in community settings including but not limited to nursing facilities, group homes, and libraries to complete Health Plan assessments and assist members in care coordination.Working with a Managed Care Organization under the oversight of DHS and completing required documents including but not limited to Health Risk Assessments, Support Plans, and other applicable assessments.Tracking a caseload of 200+ members, organizing daily actions, and member follow up to remain in compliance with DHS and program requirements.Partnering with a team to identify and implement areas of process improvement specifically related to serving members on a highly regulated health plan.Working with Minnesota Health Care Programs.Navigating and connecting residents to Hennepin County social services, including housing programs, financial assistance resources, and community-based supports.Promoting equity and inclusion for people from diverse racial backgrounds, genders, LGBTQIA+ communities, and people with differing abilities.Using Electronic Health Records, including EPIC, Guiding Care, MnChoices, Microsoft Office Suite particularly Teams, and Excel, and other related computer applications.Knowledge of:Principles and practices of care coordination.Service referral within a county system.SNBC health plan and DHS requirements.Delta Dental plan and how to connect community members with preventive and surgical dental care.Quality management and quality improvement efforts that are part of a health plan and how that relates to member engagement.Information systems relevant to care management, coordination, and reporting.The unique challenges experienced by those impacted by mental health and chemical health problems as well as the impact of complex trauma history.Trauma informed and harm reduction approaches to care.Ability to:Work independently in various settings supporting individuals who are experiencing marginalization and distress.Balance member calls, in-person member meetings, administrative requirements, data entry, and case review while providing care coordination and meeting all state and Hennepin Health requirements.Act in an advocacy role on behalf of members by partnering with internal and external waiver managers, economic supports staff, medical staff and others involved in the care of each SNBC member.Ability to function well in unpredictable and challenging situations calling for sensitivity, flexibility, resourcefulness, and independent judgement.
