Job Location: Pawhuska - PAWHUSKA, OK 74056 Position Type: Regular Full-TimeThe Community Health Representative (CHR) acts as liaison between medical provider and client to bridge gaps between Osage/Native clients and health resources. The CHR provides transportation to Osage/Native clients within the Osage Reservation who have no other means of transportation with priority to Wah-Zha-Zhe Health Center appointments and their referred appointments. The CHR strives to improve overall health and wellness and promotes disease and injury prevention of our Osage/Native community members. The CHR complies with Osage Nation Employee Policies and Procedures and Osage Nation CHR Program Policies and Procedures. Qualifications
Required Qualifications - High school diploma or GED equivalent - Certified Nursing Assistant (CNA) certificate - Willing and able to stand, walk, stoop, bend, kneel, pull, push and lift a minimum of 50 lbs. General
Qualifications - Must complete and pass American Heart Association BLS/Heartsaver/First Aid Instructor Training and Blood Borne Pathogens Training within 180 days of hire - Must complete and pass CHR Basic Training and CHR Advanced Training online modules within 180 days of hire - Must complete and pass National Child Passenger Safety Training within 180 days of hire - Must be culturally sensitive to the customs, traditions, language, practices, and sovereign status of the Osage Nation - Must be able to work independently with minimal direction - Must have good writing and public speaking skills - Must maintain a professional attitude and appearance that positively represents the Osage Nation - Must maintain confidentiality at all times, be knowledgeable of and strictly adhere to HIPAA laws - Requires a high level of personal integrity and ethics - Requires a high level of customer service and patient care - Requires ability to use Microsoft Office Suite programs - Requires good interpersonal communication and teamwork skills - Must pass a criminal background check - Must pass drug/alcohol testing - Must have a valid driver’s license and clean driving record – may be required to drive GSA or Nation vehicles - Must be willing and able to travel
