Position Type: Full Time - 30 - 40 hrs/weekEducation
Level: High School/GED - Required
Salary Range: $20.95 - $24.09 Hourly

Travel Percentage: Road WarriorThe Community Health Representative (CHR) directly impacts individuals, families, and the community by working with the CHR Supervisor, Community Health Coordinator, and medical care team to educate, promote, and support healthy lifestyles. This position maximizes the effective utilization of available health care, health education, disease prevention, dental, mental health, and social services. The position operates under the direct supervision of the CHR Supervisor and the guidance of the Community Health Coordinator.
Duties Include:
Educating:
- Assists individuals and families in using health facilities, resources, and preventative and curative practices and services in a timely, effective, and appropriate manner.
- Informs community members of the services available at the S’Klallam Health Center and other resources in our area and provides appropriate informative materials.
Home/Hospital Visits:
- Provides home visits and limited home health care for PGST community members hospitalized, physically or mentally disabled, or homebound, as directed by Tribal Health Clinic staff and outside medical providers.
Resource Access Assistance:
- Assists individuals in interpreting eligibility requirements, completing necessary forms and applications, and solving problems with various health and social services agencies and providers.
Patient Treatment and Self-Care:
- Assists patients with the proper use of medication and implementation of treatment and self-care plans as directed by Health Services and outside medical providers.
- Provides consistent, ongoing contact with patients with chronic health problems who are confined to nursing or extended care facilities.
- Assists elders in implementing exercise, walking, and stretching routines outlined by health care providers.
Medication distribution:
- Pick up and deliver all prescriptions to eligible community members (elders, disabled) by established policies and procedures under the guidance of the Community Health Coordinator.
Transporting, Advocacy, and Liaison:
- Transports patients to health care facilities, including PGST facilities, as necessary, under established policies and procedures.
- Accompanies patients to health providers, facilitates communication, and acts as a patient advocate.
- Responsible for using the Paratransit Services contract. This includes tracking patient eligibility, completing and submitting billing forms, and participating in agency audits.
Meetings:
- Attends and participates in various community meetings and staff committees whose goals directly or indirectly relate to the objectives of the S’Klallam Community Health Representative program.
Health promotion:
- Coordinates and prepares information and topics for community health fairs and other tribal events focused on health education, exercise, and other disease prevention activities.
Personal Contacts:
- Maintains regular positive contact with elders, staff, and community members and effectively liaises with Local, State, and Federal partners to accomplish PGST Health Center’s objectives and preserve good public relations.
Reporting:
- Produces and distributes written and verbal reports to Health Clinic staff and outside providers promptly as directed.
- Report concerns or abnormal findings to the Community Health Coordinator and medical team to meet the patient’s needs and to coordinate appropriate services and resources.
- Reporting is mandatory for tribal employees and
contractors with the tribe who perform services to the community in the
areas of education, health and human services, law enforcement and
members of the clergy.
Remote Work:
- This position is not eligible for remote work.
Education and Certifications:
- High School Diploma or GED Required.
- CHR certified through Indian Health Services- Required within 6 months of hire.
- Drivers License – Required
- Clean Tribal/State driving history report- Required
- First Aid and CPR certification Required.
- Defensive Driving Certification - Required within 3 months of employment.
- Naloxone training completed within 3 months of hire.

Experience:
- Utilize Tribally-owned vehicles to provide transportation to health care appointments and other transport as assigned.
- Good communication skills and will seek input from the Health Center leadership team when challenging situations arise that may require management guidance (e.g., inappropriate patient behavior).
- Personal computer skills, including proficiency with Microsoft Office, are required. E-mail and Internet skills are also required.
Physical Requirements:
- Regularly required to finger, handle, reach with hands and arms
- Frequently required to walk, sit, stand.
- Occasionally required to lift and move up to 25 pounds.
- Normal audio and visual acuity
Travel Requirements:
Local, Statewide, and out-of-state travel may be required.
