The Community Health Registered Nursepositionensures individuals who face barriers to mainstream healthcare receivetimely, respectful,evidencebasednursing support that improves health outcomes and strengthens pathways into ongoing care.

About the role

As a Community Health Registered Nurse, youwill have theopportunity to deliver comprehensive nursing care to people attending Access Health, the Access HealthVanand the St Kilda 24/7 Needle & Syringe Program. You will complete intake and assessment for new clients, support existing service users with their health needs, and work collaboratively withcolocatedclinicians to provide integrated,personcentredcare.

Reporting to the Harm Reduction Program Manager, this is a Permanent Full-Time role based onsitein St Kilda, VIC

Conditions

Compensation is in accordance with the Nurses Award - Level 2.

How you will make an impact

  • Provide frontline healthcare to people accessing Access Health, the Access HealthVanand the St Kilda 24/7 Needle & Syringe Program
  • Strengthen referral pathways between outreach programs and Access Health GP services
  • Deliverharmreductionnursing usingnonjudgemental,evidencebasedapproaches that keep clients at the centre of care
  • Complete intake and triage for new clients and provide secondary oversight to ENs
  • Support clinical operations including LAIB administration, psychotropic depots, phlebotomy,immunisationsand referralfollowups
  • Provide holistic clinical responses including assessment, care planning, coordination, supportedreferraland health promotion
  • Engage flexibly in outreach settings, public spaces, meals programs anddropinenvironments
  • Contribute to continuous improvement through program evaluation, reflectivepracticeandidentifyingservice gaps
  • Build sector capability by offering secondary consultation, triagetrainingand clinical incident response support
  • Maintainhighqualitydocumentation aligned with funding and legislative requirements

What you will bring

  • A tertiary qualification as a Registered Nurse (RN Div 1)with current AHPRA registration
  • Demonstrated understanding of homelessness, druguseand mental health
  • Knowledge of relevant nursing legislationand ability to apply it in practice
  • Experience working with vulnerable people in community settings
  • Immunisation certification(desirable)
  • Sexual health nurse certification(desirable)
  • Acurrent Employee WorkingWithChildren Check, and VictorianDriver’s Licence

The Royal Children's Hospital (RCH) has been providing outstanding care for Victoria's children and their families for over 147 years.

We are the major specialist paediatric hospital in Victoria and our care extends to children from Tasmania, southern New South Wales and other states around Australia and overseas.

With a passionate, highly skilled and committed staff campus wide of over 5,000, we provide a full range of clinical services, tertiary care and health promotion and prevention programs for children and young people.

We are the designated state-wide major trauma centre for paediatrics in Victoria and a Nationally Funded Centre for cardiac and liver transplantation.

When it comes to training and research we partner with the very best. Our campus partners, the Murdoch Childrens Research Institute (MCRI) and The University of Melbourne Department of Paediatrics, along with the RCH Foundation, are on site with the hospital in Parkville.
