Examples of duties
This role focuses on providing skilled nursing guidance, in-home evaluations, and support for care managers working with older adults in the community. The position includes both office and field responsibilities that contribute to safe, effective services for elders. As a Community Health Nurse 2, you will perform the following duties:
- Nursing Guidance: Offer consultation to staff regarding client cases across multiple programs
- Health Assessment: Conduct in-home evaluations and determine functional eligibility in various settings
- Safety Review: Identify health or safety hazards in homes and take appropriate corrective steps when feasible
- Team Collaboration: Participate in multidisciplinary meetings and provide medical input as needed
- Education Support: Provide training and information to staff, clients, and community members
- Record Management: Maintain required documentation, data, and reports in accordance with regulations
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
- Telework: You may have the opportunity to work from home (telework) part-time after six months, depending on operational needs. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Eddystone. The ability to telework is subject to change at any time. Additional details may be provided during the interview.

Qualifications
QUALIFICATIONS
Minimum Experience and Training Requirements:
- Two years of community health nursing experience and a bachelor's degree in nursing, including or supplemented by the required number of credit hours of community health nursing education, accredited by the National League for Nursing (a master's degree in nursing or public health accredited by the National League for Nursing or the American Public Health Association may be substituted for one year of community nursing experience; or
- Any equivalent combination of experience and training.
Special Requirement:
- Possession of a current license to practice as a Registered Nurse issued by the Pennsylvania State Board of Nursing; or possession of a non-renewable temporary practice permit issued by the Pennsylvania State Board of Nursing.
Condition of Employment:
- Employees possessing nonrenewable temporary practice permits must obtain licensure as a Registered Nurse within the one (1) year period as defined by the Pennsylvania State Board of Nursing.

- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
