Department: Center Staff

Reporting To: Kin Yee Ho

Compensation: $24.00 - $29.00 / hour





Under the direction and supervision of the Healthy Living Program Director, the Part-time Community Health Instructor will conduct safe, effective, and educational wellness classes designed specifically to meet the needs of our community members. The Community Health Instructor will promote community health, both internally and externally, for the YMCA of Greater Boston. The candidate will commit to and support the Y mission and vision to advance community-integrated health. Promote the Y’s ability to serve health seekers in preventing chronic disease or living well with chronic disease. Support the Healthy Living Program Director in achieving all the chart of work deliverables necessary to build the organization's capacity to launch one or more community health programs.

Additionally, this role will coordinate community outreach and engagement efforts , as well as support the implementation and delivery of four core community health programs, ensuring culturally responsive, accessible, and high-impact services for diverse populations.

Key Responsibilities

KEY RESPONSIBILITIES: Community Outreach & Program Coordination


The Part-time Community Health Instructor will support outreach, recruitment, coordination, and delivery of the following programs:

  • Livestrong: Exercise training and peer-support programs for cancer survivors.
  • Blood Pressure Self-Monitoring: Education and tracking support program that teaches those with or at risk for hypertension to build the practice of regularly monitoring their blood pressure and make recommended dietary changes.
  • Healthy Habits: Physical and mental health programs that focus on building the 5 key habits proven to be associated with a healthy lifestyle, including stress reduction, sleep, social connection, nutrition, and exercise.
  • Enhance Fitness: Functional Fitness training and social engagement for older adults.


ESSENTIAL FUNCTIONS:

  1. Facilitate and/or co-facilitate chronic disease and prevention programs in accordance with established guidelines.
  2. Must be able to maintain confidentiality and adhere to HIPAA laws.
  3. Lead in-person or virtual classes and takes responsibility for group dynamics and interactions among members.
  4. Keeps attendance and completes associated paperwork following each session.
  5. Exceptional listener and engage in active listening with participants.
  6. Must be able to express empathy to participants.
  7. Provide curriculum to class participants in effective, meaningful, and compelling ways.
  8. Encourage group participation and interaction with open-ended questions, facilitate commitment and retention of participants.
  9. A good knowledge of healthy behavior and the difficulties associated with making a lifestyle change.
  10. Appropriately provide and implement sessions in accordance with National and State standards and guidelines.
  11. Delivers each program with the curriculum as set forward by YMCA of the YUSA or licensing organization.
  12. Responsible for the health and safety of individuals during each session.
  13. Report accidents and injuries as well as complete incident reports.
  14. Set up and break down equipment as needed.
  15. Create a supportive and motivating environment that is friendly, fun and non-competitive.
  16. Foster relationships with and between participants.
  17. Make learning a shared responsibility for the group.
  18. Provide accessibility to participants both before and after sessions to answer questions and follow-up on any questions you cannot answer during class time.
  19. Arrive for class on time and dressed appropriately.

Skills, Knowledge & Expertise

QUALIFICATIONS:


· Experience implementing evidence-based program models, including targeting specific or underserved populations, managing referral relationships with health care providers, and implementing quality assurance and data collection protocols.

· CPR/AED certification

· Associate or bachelor’s Degree

· Public health background, a plus.


ADDITIONAL QUALIFICATIONS: (After hire)


· Attend Orientation to Healthy Living at the Y

· Attain EBHI certifications in appropriate courses through YUSA

· Attain appropriate health and wellness certifications through YUSA or an outside agency approved by the YMCA of Greater Boston
