The Community Health Analyst coordinates with community organizations to prevent disease and promote wellness. The position analyzes primary and secondary public health data and leads SCHD and community partners with the planning, implementation and monitoring of the Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP). Employee may deploy to the field as a responder or in a leadership position during public health emergencies.

Lead SCHD and coordinate partner activities for the Community Health Assessment (CHA)

  • Lead community assessment activities, including community surveys and partner meetings
  • Coordinate data collection with internal staff and community partners
  • Update and revise the CHA with new information or data from partners
  • Communicate results of the CHA to community partners
  • Write CHA reports for internal and external partners

Monitor and lead the implementation of the Community Health Improvement Plan (CHIP)

  • Build and maintain relationships with internal and external partners
  • Coordinate partner agencies in the development of the multi-year CHIP
  • Work with community groups to implement strategies identified in the CHIP
  • Monitor CHIP activities reported by community partners
  • Maintain CHIP data reporting system
  • Write CHIP reports for internal and external partners
  • Drive to meetings with community partners

Collect, analyze, and report primary and secondary health data

  • Maintain the electronic health assessment data tracking system
  • Accurately update, analyze, and report required information on health status indicators
  • Collect and analyze chronic disease data and communicate results to internal Health Department staff, external health partners, and the public through presentations, written reports, scientific posters, and peer-reviewed journal articles
  • Respond to data requests from individuals and organizations that request data

Minimum Qualifications: Bachelor’s Degree in a related field such as public health; public administration; a biological, medical or health science; or communications. Three credit hours of coursework in epidemiological methods, data analytics and/or statistics. One year of data analysis and data management experience. Experience working with community organizations. this is a driving level position that requires a valid US driver’s license without restrictions and current proof of automobile insurance.

Must complete Federal Emergency Management Administration (FEMA) IS 100, 200, and 700 courses within 30 days of employment.

Preferred Qualifications: Five years of experience in public health, including community assessment and program evaluation. Three years of data analysis and data management experience. Two years of experience working with multiple community organizations. Master of Public Health or related masters such as public administration, biological, medical, health science, or communications. Bilingual in English and Spanish or Vietnamese.
