
Ohel is looking for Community Habilitation Counselor who are creative, dedicated, and responsible to work one-on-one with youth who have mental health challenges or who are medically fragile and live at home in the community. Community Habilitation covers in-person services and supports related to the youth’s acquisition, maintenance, and enhancement of skills necessary to perform Activities of Daily Living (ADLs) and/or Health-Related Tasks delivered in the community.
Responsibilities include addressing health and safety needs required during the delivery of the service, and the necessary staff actions to assist the youth in reaching their Community Habilitation outcome. The activities are designed to preserve or enhance independence or reduce the loss of independence.
Requirements:
Valid NY or NJ Driver’s license preferred. Good English communication skills a must. Flexible part time, after school hours. Locations include Brooklyn, Queens, Far Rockaway and Long Island.
Salary: $19.00-$22.00 per hour
