We’re now looking for a Community Enrolled Nurse (EN) to join our growing team, supporting care delivery through virtual clinical oversight and collaboration with frontline teams.

About the Role

This is a unique opportunity to step into a community-focused, largely
virtual nursing role, where you’ll play a key part in maintaining clinical
quality and safety across Support at Home services.

You’ll work closely with care partners and internal teams to review
clinical documentation, identify risks, and ensure care remains responsive to
changing consumer needs, all while supporting best-practice delivery in a
flexible work environment.

Key Responsibilities

Clinical Oversight & Review

  • Conduct remote reviews of care plans, clinical notes, and assessments
  • Identify changes in health status, mobility, cognition, and overall
    wellbeing
  • Ensure documentation aligns with clinical standards and Support at
    Home requirements

Risk Identification & Escalation

  • Detect early signs of deterioration or unmet needs
  • Escalate concerns and coordinate timely reassessments or referrals
  • Support proactive care to reduce hospitalisations and risks
  • Provide guidance to care partners and frontline staff
  • Support consistent, high-quality clinical practice across services
  • Assist with documentation standards and care planning

Collaboration & Reporting

  • Prepare clear clinical summaries and reports
  • Work closely with nursing, allied health, and quality teams
  • Contribute to audits and continuous improvement initiatives

About You

You’re a confident and proactive Enrolled Nurse who thrives in a community or virtual care environment.

  • Current AHPRA registration as an Enrolled Nurse
  • Experience in aged care, community care, or home care services
  • Strong clinical assessment and documentation review skills
  • Ability to work autonomously in a remote setting
  • Sound understanding of escalation pathways and multidisciplinary care
  • Highly organised with strong attention to detail
  • Passionate about person-centred, evidence-based care
