Job Description

The PRN Community Engagement Specialist is responsible for representing Transitions LifeCare (TL) in the community, strengthening relationships with key community partners, and supporting outreach, education, awareness, and engagement activities across our 11‑county service area. This role blends community presence, event support, outreach relationship-building, and communication support to expand TL’s reach and ensure meaningful connections across diverse communities. The schedule for this position is 10-15 hours per pay period- evenings and weekends.

Responsibilities what you do

  • Represent TL at community events, health fairs, faith‑based gatherings, partner meetings, and other local engagements.
  • Serve as a consistent and culturally competent presence in underserved, rural, and diverse communities.
  • Build and maintain relationships with community organizations, civic groups, senior care providers, and local leaders.
  • Assist in planning and delivering outreach activities, educational sessions, and special events as needed.
  • Coordinate logistics, materials, and communication for events and marketing activities.
  • Support social media and communications efforts with content ideas, photos, and community stories.
  • Share community insights and feedback to inform TL’s strategy and program development.
  • Maintain accurate records of community interactions and engagement activities.
  • Perform administrative duties and support special projects as assigned.

  • Promptly communicate concerns, issues, or potential risks to your immediate supervisor or another member of the management team to support a safe and effective work environment

Requirements what you need

  • Bachelor’s degree preferred (communications, public health, social work, marketing, or related field).
  • Minimum 2 years’ experience in community outreach, health‑related services, or customer engagement.
  • Strong verbal and written communication skills.
  • Ability to engage diverse communities with cultural sensitivity.
  • Strong organizational and administrative abilities.
  • Competence with Microsoft Office applications.
  • Ability to work independently and collaboratively.
  • Access to transportation with abilityto travel across the TL service area.
  • Ability to lift upto 30 pounds and support event setup activities.
  • Must have sensory abilities to communicate with community contacts and other members of the clinical and administrative teams and to complete the reporting and follow-up documentation responsibilities of the job.
