Job Description
Catholic Social Services is currently seeking a part-time 25 hours per week, Community Disability Counsellor in EDMONTON to join our St. Aidan. Providing excellent client-centered care, you are confident, thoughtful, and motivated to achieve success when supporting others in your community.
This position has a weekly schedule of: Monday to Friday 8am-1 pm.
The Community Disability Counsellor is responsible for working with people with developmental disabilities in their homes. The majority of the time in this role is spent assisting the people in the home to access the community, complete their daily living skills and connecting with natural supports. This position has additional administrative requirements that support the smooth delivery of service to the people living in the home such as report writing and liaising with external stakeholders.
Job Duties:
• Monitor and report changes in individuals’ health, behaviour, and overall well-being.
• Support the development and implementation of behaviour support plans for individuals with behavioural care concerns.
• Develop annual service plans and assist with data collection to support service planning.
• Implement service plan goals and teach programs, guidelines, life skills, and self-help skills.
• Support individuals in accessing community recreation, leisure activities, and community resources.
• Arrange transportation and schedule appointments, assisting individuals with attending appointments as required.
• Ensure individuals’ basic needs are met, including food, clothing, shelter, healthcare, and safety.
• Assist with personal care routines and perform safe transfers using appropriate lifting and transferring techniques.
• Administer medications as directed and maintain accurate medication records and binders.
• Respond appropriately to emergency situations and perform Fire Marshall duties as assigned.
• Support individuals in developing social, community, and independent living skills.
• Maintain the home and equipment according to Agency standards, including cleaning, laundry, dishes, and general household tasks.
• Perform lawn and yard maintenance, including mowing grass and clearing snow from walkways.
• Follow infection prevention and control practices when completing household and equipment cleaning.
• Report maintenance needs, equipment concerns, damage, and other issues to the Team Leader or on-call Supervisor.
• Follow prepared menu plans, purchase groceries and supplies, and prepare meals.
• Manage individuals’ funds and complete banking duties as required.
• Complete accurate shift reports, log notes, incident reports, serious occurrence reports, statistics, and other required documentation.
• Use on-site computers and electronic systems appropriately and effectively.
• Complete annual inventories and maintain accurate filing of documentation.
• Order and receive medications through the web-based medication delivery system and communicate with the pharmacy regarding individuals’ ongoing medication needs.
• Plan and coordinate activities based on individuals’ needs, interests, and service plans.
• Communicate concerns and suggest solutions to the team and Supervisor.
• Attend staff meetings, training, and professional development programs.
• Support team members through effective communication, collaboration, and participation in team initiatives.
• Model appropriate behaviour while on shift, including appropriate use of technology, and promote positive public relations.
• Make effective use of supervision and feedback provided.
What This Job Requires:
• Diploma/Degree in Human Services or equivalent
• Minimum 1 year experience working with persons with developmental disabilities, behavior, mental health & addictions
• Experience with or willingness to be trained on physical holds on the individuals that we serve
• Nonviolent Crisis Intervention Training (NVCI) must be completed upon hire
• Must have experience working with behaviors of concern
• Program development experience is considered an asset
• Should have good experience in personal care, as well as lifting
• Vulnerable Sector Check issued by RCMP or City Police
• Child Intervention Record Check
• A reliable Vehicle, Valid Class 5 Driver’s License, Vehicle Registration and Third Party Liability Insurance (amount of two million)
• 2 Years of Driving experience, Driver's Abstract with less than six (6) demerit points current within one (1) year
