Implement and administer necessary and appropriate transition services for students with special needs who have socially graduated and have chosen to continue accessing secondary services. Typically these are students 18-21 years of age. Provide oversight of transition services and IEP compliance issues for high school students with special needs. Requirements of the Transition Specialist position are governed by the transition regulations found in the Individuals with Disabilities Education Act (IDEA).

Essential Duties and Responsibilities:

• Provide case management for 18-21-year-old students with special needs and monitor progress toward post-secondary goals and objectives.
• Serve as liaison among adult service providers, students, families, and SPED staff. Provide consultation services for students, families, and APED staff as necessary. Collaborate with various community agencies and adult services providers to provide services and support for students and families.
• Attend all senior IEP meetings at Harrison High School and Sierra High School to assist in the development of students' post-secondary pursuits.
• Assist and/or train secondary SPED staff in IDEA compliance issues and transition best practices.
• Assist the Director, Assistant Director, and/or Coordinator of Special Programs in completing any departmental assignments of importance.
• Participate on the Pikes Peak Interagency Transition Team to gather necessary resources and information to share with secondary SPED staff. As a member of the Pikes Peak Interagency Transition Team represent D2 by contributing to community activities and issues that are of significance to students with disabilities.
• Facilitate and participate in the STEPSS (State Toolkit for Examining Post School Success) model.
• Participate in continuing education and professional growth activities related to transition.
• Perform other duties as may be assigned by supervisor/administrator.

• Valid Colorado License issued by the Colorado Department of Education with Special Education Teacher endorsement or BCBA endorsement (currently issued as an Adjunct Instructor Authorization).
• The Colorado License/Authorization must be held within 30 days of the candidate's start date in this position.
• Each special education provider shall serve, at a minimum, a majority of students whose identified area of need aligns with the provider's endorsement.
• Endorsement levels must be appropriate for the age range being served, in accordance with ECEA Rule 3.04.
• Bachelor's Degree. Special experience and training as determined by content area.
• Criminal background check and District fingerprinting required.

• In-depth knowledge of content area.
• Good organization and communication skills.
• Ability to communicate effectively with stakeholders within the school environment.
• Knowledge of classroom management and conflict resolution.
• Knowledge of and experience in Professional Learning Communities.
• Ability to walk or stand for long periods; routinely lift and carry up to 30 pounds; occasionally lift and carry up to 50 pounds.
