
Kendal at Hanover is a non-profit Continuing Care Retirement Community and home to 400 residents.
Kendal at Hanover is seeking a Community Care Manager (RN or LPN)to provide support and oversight for resident care coordination, physical and psychological wellbeing, and transitions in care for residents across the Kendal community. The primary focus of this role is ensuring residents are residing in the appropriate level of care and collaborating with them and their interdisciplinary team when transitions are needed, including short-stays, respite stays, direct admissions, and permanent transitions to higher levels of care. As one of the Nurse Supervisors on duty, this role also provides supervision and support to nursing staff and may assume direct nursing care duties including medication administration and wound care.

The hours for this position are Monday through Friday from 8:00 AM - 4:30 PM.
