Te whiwhinga mahi: The opportunity

Are you passionate about championing Māori health equity and making a real difference in the lives of whānau? We are looking for a Community Care Coordinator to join our innovative, cross-agency partnership in the Tairāwhiti region.

This is a unique opportunity to work across Pinnacle Midlands Health Network, Te Rūnanga o Tūranganui a Kiwa, and Ngāti Porou Oranga, connecting whānau to the health and social services they need to thrive. Rather than sitting within a single team, you will work fluidly across the health sector, building relationships and coordinating wrap-around care.

In this role you will:

  • Champion Māori health and equity, prioritising whānau with the greatest need.
  • Coordinate care across health and social services for individuals, whānau, and communities.
  • Facilitate smooth transitions and continuity of care across providers.
  • Identify and support those at greatest risk of poor health outcomes.
  • Act as the voice of primary health within the partnership, supporting Iwi aspirations.
  • Work collaboratively across agencies to deliver whānau-centred care.

This is a full-time (40 hours per week) fixed-term role until 30 June 2027.

He kōrero mōu: About you

  • Registration as a health professional (e.g. registered nurse, physiotherapist, occupational therapist, social worker) with a current annual practising certificate.
  • Primary or community healthcare experience.
  • A strong understanding of health inequities in Aotearoa New Zealand and a commitment to achieving equitable outcomes for Māori.
  • Excellent communication, time management, and organisational skills.
  • Commitment to Tikanga Māori and Te Tiriti o Waitangi.
  • A current full driver’s licence.
