Role Overview

The Community Care Coordinator conducts annual resident assessments and service plans, coordinate referrals and community resources, facilitate educational and wellness programs, manage resident concerns and crisis situations, monitor service delivery and housing placement needs, collaborate with multidisciplinary teams, and continuously enhance skills to support resident well-being and organizational success.

Job Responsibilities

  • Conduct annual assessments of the residents' needs using the prescribed resident application. Research and maintain a network of service providers in the community. Assist residents in the development of their annual service plans, including but not limited to providing referrals with professionals standards, and external requirements of a relevant regulatory, licensing or accrediting bodies. Facilitate events and educational presentations for the residents and their families on topics including but not limited to health, wellness, safety, financial assistance, and residents rights.
  • Handle all resident and visitor complaints as well as crisis situations in consultation with their supervisory chain in compliance with CCAC/Programmatic policies and procedures and treats each case with sensitivity and expedience.
  • Coordinate the annual building assessment to identify residents who are frail and/or at risk. Assist residents in the development of annual service plans. Provide referrals to service agencies and service providers to the residents and monitor service delivery. Assist management when necessary to permanently place a resident in a more appropriate setting.
  • Collaborate with multidisciplinary teams to coordinate services and ensure comprehensive care.
  • Engage in ongoing learning, training, and skill development, seeking and applying feedback to continuously improve performance and contribute to team and organizational success.

PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.

  • Kneel and move from sitting, bending, kneeling, or standing multiple times a day
  • Climb up and down up to 3 flights of stairs at a time
  • Lift, push, and pull up to 10 lbs

Education and Experience Requirements:

Minimum

  • Associate's Degree (AA) in a related field of study 1-3 years relevant experience
  • Strong customer service skills, able to work well with a diverse population, great communication skills.
  • Previous experience completing assessment and evaluations.

Preferred

  • Bachelor's Degree in Arts/Sciences (BA/BS) in a related field of study 3-5 years relevant experience
  • Previous experience completing assessment and evaluations specifically within the senior population.
  • Data entry experience preferred.

Salary

Starting at $23.30 / hour

In compliance with the Illinois Equal Pay Act of 2003 (820 ILCS 112/5, et seq.), Catholic Charities of the Archdiocese of Chicago is required to provide the wages and benefits that it reasonably expects in good faith to offer for any specific job positions posted. Please note that the exact compensation package ultimately offered to an applicant may vary based on skills, experience, and location, amongst other permissible factors.
