
The Palliative Medicine Nurse Practitioner provides consultation in palliative medicine to meet the needs of a patient as requested by their referring physician. The nurse practitioner is a specialist in assessing the patient's condition and developing a plan of care to improve patient understanding of their complex illness, adherence to course of treatment, and quality of life in context of patient’s values. The nurse practitioner also develops a plan of care to manage complex disease-related symptoms including emotion, physical, and existential components. Importantly, the nurse practitioner communicates with all members of the patient’s interdisciplinary care team to insure proper coordination of the best care possible.

KeystoneCare offers post-hospitalization home care, palliative care, comprehensive hospice services, private duty care, and telehealth services. For more than 20 years, we have helped improve the quality of life for our patients and their families with comfort and dignity, regardless of stage of life or state of health. KeystoneCare is also known as Keystone Hospice and KeystoneCare Home Health.
KeystoneCare is Medicare and Medicaid certified, and maintains contractual agreements with most of the major insurance plans in the region. We are also a participant in the Pennsylvania Department of Human Services' Aging Waiver Program. KeystoneCare is governed by a Board of Directors.
The Keystone Homecare and Hospice teams are comprised of social workers, certified nursing assistants, licensed practical nurses, registered nurses, nurse practitioners, medical director, physical therapists, occupational therapists, chaplain, and a certified music therapist. Keystone House, the residential hospice in the historic John Welsh House on Stenton Avenue in Wyndmoor, Pennsylvania, has a support staff that also includes volunteers from the local community.
