Job Description

 Communicable Disease Investigator and Health Educator - (261001-50025890)

Purpose of Position: To perform case investigation, surveillance, and follow up with the Massachusetts Department of Public Health (DPH) via the Massachusetts Virtual Epidemiologic Network (MAVEN) for our contracted towns. To provide community educational presentations and exercise programs upon request.

Description

: Performs all tasks related to case investigation for the VNA contracted Towns via MAVEN Participates in all local and state level meetings/trainings to stay apprised of updated guidance Communicates the status of disease patterns and trends to local community partners Develops and presents educational presentations on various health topics upon request Leads community programs upon request Supports the Department with administrative duties as needed Consistently provides service excellence to all patients, family members, visitors, volunteers and co-workers. All other duties assigned

Qualifications

: Bachelor’s degree recommended, Public Health or related major is ideal Excellent oral and written communications skills a must Experience in customer service a plus Must be a critical thinker and be able to handle confidential information with discretion Excellent computer knowledge

Schedule Details: Full-Time, Monday-Friday, Occasional Weekends & Holidays
