JOB TITLE: Certified Nurse Aide (CNA)

POSITION SUMMARY:
The Hospice Certified Nursing Assistant (CNA) provides compassionate, hands-on care to patients in the home setting, supporting comfort, dignity, and quality of life. Responsibilities include assisting with personal hygiene, grooming, dressing, and mobility, as well as performing vital signs, weights, and simple dressing changes as directed by the RN. The CNA helps with medication assistance, nutritional support, light meal preparation, and light housekeeping in accordance with the patient’s care plan.
This role requires close collaboration with the RN Case Manager and interdisciplinary team, timely documentation of care, and reporting changes in patient condition. The CNA ensures safe use of equipment, maintains cleanliness, and reinforces patient and caregiver education provided by the RN. Compassion, attention to detail, and commitment to patient-centered care are essential for success in this position.

INTERPERSONAL RELATIONSHIPS:
All comments, actions and behaviors have a direct effect on the public’s perception of Hospice Austin. Interactions with patients, family members, physicians, referral sources, visitors, co-workers, etc. must be in a manner that is courteous, respectful, cooperative and professional. This behavior should promote an atmosphere of teamwork and positive relations.
DUTIES AND RESPONSIBILITIES:
- Assists with personal hygiene--baths, grooming (hair and nails), provides oral hygiene, checks skin integrity.
- Provides peri-care and performs Foley care and maintenance.
- Dresses patients.
- Performs patient transfers, turns, etc. while ensuring safety of patient and self.
- Takes vital signs and weights as directed.
- Ambulates patients as appropriate and does range of motion exercise as directed.
- May perform simple dressing changes as directed by the RN (i.e. non-sterile, non-surgical, and not greater than Stage 1 pressure ulcers).
- Assists with medications that are ordinarily self-administered.
- Provides care consistent with written instructions as outlined in patient’s care plan.
- Demonstrates understanding and support in interactions with team members
- Communicates with RN Case Manager and other team members as appropriate; reports changes in patient’s condition and needs.
- Documents care in a timely manner, and in compliance with organization policies and procedures.
- Obtains supplies and teaches use of medical supplies.
- Uses and reinforces the RN’s teaching on use of equipment.
- Checks equipment for safety and cleans equipment.
- Changes linens.
- Provides nutritional support, assists with feeding, light meal preparations and light housekeeping as indicated.
- Other duties as assigned.
QualificationsMINIMUM TRAINING, EDUCATION AND EXPERIENCE REQUIREMENTS:
- Current, active Texas nurse aide certification (CNA).
- Currently listed in good standing on the Texas nurse aide/misconduct registry, with no finding against the aide relating to client abuse or neglect or misappropriation of client property, as verified through the Texas Department of Human Services (DHS).
- Valid Texas driver’s license, auto liability insurance and safe driving record.
- CPR certification.
PREFERRED QUALIFICATIONS:
- Hospice or home health experience.
