What We Are Looking For

The CMT Training Specialist plays a crucial role in enhancing the skills and knowledge of Care Management staff within the outpatient setting. Collaborating closely with CMT leadership, this specialist is tasked with designing and executing a comprehensive training program. The program encompasses crucial aspects such as outreach, engagement, documentation, patient support, and program deliverables. Delivered exclusively in the outpatient setting, the training program incorporates on the-job training, refresher courses, and the orchestration of external training mandated by the program. A key responsibility of this role is to ensure that staff members are well-equipped and trained to fulfill organizational, programmatic, and contractual requirements effectively within the outpatient context. This proactive approach contributes to the overall success and excellence of the Care Management team in the outpatient setting.

ESSENTIAL FUNCTIONS AND DELIVERABLES

  • Collaborate closely with management to craft training programs aligned with program guidelines and contractual requirements.
  • Conduct comprehensive new employee training sessions, covering crucial areas such as chart review, enrollment, assessments, documentation, registration, and program deliverables.
  • Oversee external training obligations for new employees, including APS/CPS reporting, mandatory payors' learning management systems training, community health worker training, motivational interview training, and more.
  • Manage employee access to Electronic Medical Records (EMR) and Case Management systems, ensuring completion of required training within internal departments and external organizations.
  • Provide insightful feedback to leadership on technology updates or adaptations needed to meet new program requirements. Collaborate with leadership in creating and developing systems essential for program compliance.
  • Deliver reinforcement and skill-building sessions to CMT staff based on management feedback regarding employee performance.
  • Monitor and administer skills competencies, with remediation as necessary to ensure continuous improvement.
  • Actively contribute to the development of workflows and guidelines based on program updates and new contracts.
  • Conduct thorough chart reviews to uphold quality assurance and adherence to workflows. Communicate training gaps to management and collaborate on finding effective resolutions.
  • Maintain proficiency in key applications such as Epic, Care Director, IEHP Portal, Manifest Medex, and any other Care Management-related systems essential for program functionality. May assist with system support, enhancements, upgrades, and implementations.
  • Stay current on Care Management program details, remaining up-to-date on program, contract, and grant deliverables to ensure a thorough working knowledge of the field.
  • Travel to other SAC Health clinics as necessary; must have a reliable vehicle, valid
  • driver's license, and auto insurance.

QUALIFICATIONS:

  • Education: High School Diploma or equivalent required.
  • License/Certification: Must have an active Medical Assistant or Community Health Worker Certification. Current CPR/BLS certification (must be American
  • Heart Association or Red Cross accredited program). Valid California driver's license, and auto insurance.
  • Experience: Minimum 1+ years of experience within the SAC Care Management program, along with 1+ year of experience as a Medical Assistant (MA) or Community Health Worker (CHW) specializing as a Patient Navigator. Previous training experience is essential for this role.
  • Essential Technical/Motor Skills: Demonstrate expertise in CMT procedures, recognizing the need for supervision or consultation when required. Showcase
  • precise motor skills for hands-on training and proficiency in Health Care Information Systems, including Electronic Health Records. Possesses advanced
  • computer skills in Microsoft Word, Excel, and Access for effective CMT training. Stay adaptable to evolving technologies and remain receptive to guidance.
  • Interpersonal Skills: : Exhibits positive interpersonal relations in interactions with various stakeholders, including fellow trainers, directors, and external agencies. Maintains effective communication channels with trainees, ensuring updates on CMT procedures, addressing concerns, and providing guidance. Demonstrates clear communication to internal teams, fostering collaboration and problem resolution. Regularly engages with external partners to enhance communication on CMT advancements and opportunities for improvement.
  • Essential Mental Abilities: Shows strong critical thinking and problem-solving skills in CMT contexts. Analyzes complex information, adapts training approaches, and quickly grasps new developments, ensuring effective and up-to-date training delivery.
  • Work Eligibility: Must be legally authorized to work in the United States on a full-time basis. Must not now or in the future require sponsorship for employment visas.
