Employment Type: Part Time 0.9 FTE

Fixed-term contract based on contract funding through to 30 October 2027

The Clozapine Coordinator supports the delivery of the Clozapine Clinical Care Coordination service across the South West region, assisting General Practitioners (GPs) and authorised medical
providers in the management and monitoring of people receiving Clozapine pharmacotherapy.

The role provides clinical care coordination, mental and physical health monitoring, medication support and service navigation for individuals receiving Clozapine treatment, particularly during transition from inpatient care to community-based support. The position works collaboratively
with consumers, carers, families, GPs, hospitals and community services to support safe, coordinated and recovery-oriented care, while improving access to services that address mental health, physical health and broader wellbeing needs.

Key Components of the Role

  • Liaise with referring primary care provider to support implementation of the individuals receiving Clozapine pharmacotherapy across the South West region.
  • Support individuals to understand and follow their GP Management plan (GPMP) and adhere to treatment regimes, for example, medication compliance, including mental and physical health monitoring and medication support.
  • Work collaboratively with consumers, carers, families, psychiatrists, hospitals, pharmacists and community services to support safe, coordinated and recovery-oriented care.
  • Support consumers to access appropriate mental health, physical health and community support services to improve wellbeing and continuity of care.
  • Arrange and facilitate access to allied health services as identified in the GPMP and intake and assessment process.
  • Assist individuals to address barriers to engagement including referrals to appropriate services.
  • ·Encourages individuals to engage in regular health check ins with their primary care provider, supporting ongoing care and wellness.
  • Capture, coordinate and enter service delivery data with patient experience and outcome measures in accordance with WAPHA reporting requirements.
  • Develop and maintain strong collaborative relationships with health partners and service providers particularly General Practice staff and WACHS providers.
  • Participate in Oseca’s Clinical Governance activities and work within the Organisation’s Clinical Governance framework.
  • Promote culturally safe, inclusive and client-centred service delivery.

Essential Requirements:

To be considered for this role, you must meet the following criteria:

    • Registered Nurse (Mental Health notation suitable)
    • Current registration with AHPRA
    • Experience working with individuals who have been diagnosed with psychotic conditions and/or individuals receiving Clozapine pharmacotherapy, living in the community.
    • Post Graduate Qualifications in Mental Health (desirable)
    • Strong communication and interpersonal skills
    • Current Western Australian Driver’s licence
    • National Police Clearance
    • A Working with Children Check (WWC)
    • Pre-Employment Health Assessment

In the South West, Down South Aboriginal Health, Integrated Primary Mental Health Care, Integrated Chronic Disease Care, Diabetes Education, Parenting Support Program and the 3 Tier Youth Mental Health Program are provided.
