We’re seeking a passionate and experienced Clinician - Restorative Care to play an integral role in delivering clinical oversight and coordination within our Support at Home Program’s Restorative Care Pathway.

Reporting to the Head of Care - Fully Coordinated, you’ll provide multidisciplinary clinical leadership across short-term restorative episodes (up to 16 weeks), ensuring care delivery is intensive, purposeful, and aligned with participant goals focused on reablement, independence, and functional improvement.

Grounded in restorative philosophy, this role champions approaches that build or restore participant capacity - focusing on what people can do and supporting them to do more. You’ll ensure clinical governance, model fidelity, compliance, and quality outcomes across each episode of care, while supervising and supporting Restorative Care Partners.

What You’ll Do

  • Conduct comprehensive clinical assessments and establish functional baselines
  • Develop structured, time-limited restorative goal plans aligned to participant outcomes
  • Lead multidisciplinary case conferencing and coordinate restorative care episodes
  • Provide ongoing clinical monitoring, goal evaluation, and plan adjustments
  • Initiate exit planning at the commencement of each episode and document clear recommendations at completion
  • Provide clinical governance oversight and supervise Restorative Care Partners
  • Monitor documentation quality, regulatory compliance, and funding utilisation
  • Escalate clinical risks and support incident management processes
  • Contribute to quality assurance activities, audit readiness, and pathway improvement initiatives
  • Ensure alignment with the Aged Care Act 2024, Support at Home Program Manual, and Aged Care Quality Standards
  • Promote dignity of risk, supported decision-making, and culturally safe care

What You’ll Bring

  • Registered Nurse or relevant Allied Health professional with current AHPRA registration
  • Minimum 4 years’ post-graduate clinical experience
  • Demonstrated experience managing complex, multidisciplinary cases
  • Experience in rehabilitation, chronic disease management, or episode-based care
  • Strong clinical governance knowledge and risk management capability
  • Proven ability to work collaboratively across care, quality, and operational teams
  • Excellent documentation, compliance, and audit awareness
  • A restorative and reablement mindset, focused on measurable participant outcomes
  • Strong leadership capability with experience supervising or mentoring clinicians
  • High level of professional integrity and commitment to person-centred care

Desirable:

  • Experience in restorative or transition care models
  • Experience within Support at Home or Home Care programs
  • Postgraduate qualifications in gerontology, rehabilitation, or care coordination
  • Experience working within geographically dispersed teams
