Assess clients' mental health status, abilities, and ongoing service appropriateness.
Assist clients in developing and implementing individual treatment plans; regularly review and modify plans, focusing on achieving goals.
Obtain insurance authorization and complete re-authorizations promptly.
Communicate clients' needs with emergency personnel and hospital staff.
Conduct care coordination and communication activities based on needs identified in the treatment plan.

Qualifications
Master's or doctoral degree in social work, marriage and family therapy, counseling, psychology or a related mental health field required.
Clinical experience in a psychiatric or substance abuse setting preferred.
Must be proficient with PCs and Microsoft Office Word & Outlook, able to perform concurrent documentation and navigate through electronic health record.
