The Case Management Clinician works one on one or in groups with individuals who choose our program to meet their needs. They will provide support to individuals 65+ as they improve their health and quality of life in the face of Mental Health and/or Addictions concerns and the accompanying barriers they experience. The Seniors Outreach and Recovery (SOAR) Case Management Clinician assesses the person’s needs, strengths, and supports to co-develop goals and a customized recovery and support plan. You would work from a trauma informed perspective as part of a team and part of a community to advocate, challenge stigma, and prevent harm.
The Case Management Clinician acts as a special resource in psychogeriatric care, including co- occurring disorders; provides consultation and formal teaching; is responsible for the provision of clinical services to the psychogeriatric client through assessment, treatment, monitoring and directing care; act as liaison with community agencies and care providers; and assists in community development activities.
