The Clinical Care Coordinator is responsible for developing and providing high-quality therapeutic interventions and services in support of an individual’s wellness and recovery. Services include strength-based and needs-based therapeutic case management, problem-solving, therapeutic rehabilitation including skills-building, benefits support, resource development, housing navigation, crisis intervention, and family support. Schedules may include after hours as needed by the program.

The THRIVE programs are new initiatives by Sacramento County Behavioral Health to provide services specifically for adults who are justice involved and meet criteria for specialty mental health services. Justice involved may include participating in mental health diversion, mental health court, parole/probation, etc. THRIVE Outpatient Program will serve the moderate level community members and be open a standard Monday-Friday. THRIVE FSP will serve the high intensity community members and operate seven days a week.

Schedule Tuesday-Saturday (FSP), Sunday-Thursday (FSP), or, Monday-Friday (Outpatient). Day Shift.

DUTIES AND RESPONSIBILITIES:

  • Delivery of individual, family, group counseling, individual rehabilitation, and case management/brokerage for individuals within the Care Coordinator's caseload as assigned.
  • Utilize Evidence-Based Practice for the designated population/program to assure high-quality results and intended impact.
  • Assess an individual’s risk status including risk-based symptoms, substance use, environment, physical conditions, etc, and intervene appropriately.
  • Creating a service plan that aims at an individual getting better/healthy and accomplishing service goals that are person-centered and belong to the client.
  • Outreach and progressive engagement and reporting disengagement to the team immediately. Responsible for working with the outreach team and determining when to disenroll individuals based on program protocols.
  • Assessment, screening referrals, service planning, community planning, and benefits coordination support wellness and recovery principles.
  • Documents and maintains all data collection, reporting, and charting records in accordance with BACS and other relevant county regulatory policies/agencies.
  • Participates in supervision, staff meetings, consultation, travel, crisis/emergency coverage, weekend coverage, as well as, agency-wide meetings and training, as required by the manager.

QUALIFICATIONS:

  • ASW, AMFT, or APCC with current BBS registration; and 1 year of social service experience
  • LCSW, LMFT, or LPCC with current BBS registration; and 1 year of social service experience
  • Valid California driver’s license, personal vehicle, and proof of personal vehicle insurance coverage. DMV printout required.
  • Extensive local travel is required, vehicle reimbursement is provided. Clinical Care Coordinators transport clients in their personal vehicle.

  • Highly competitive compensation. $40/hourly, non-licensed but BBS registered AMFT, ACSW/APCC and $104,000 annually (exempt) for licensed BBS registered LCSW/LMFT/LPCC.
