Job Description

Under the supervision of the Clinical Director, the Clinician is responsible for ensuring that proper clinical and case management services as well as community reintegration planning are provided to the members. This role will have a large outreach component, as this person will act as a liaison between members and community connections, as they begin their process of transitioning to a more independent status within this community.

PROGRAM/DEPARTMENT RESPONSIBILITIES

· Ensure that an intake screening (suicide/safety assessment) is completed for each new intake to the program.

· Provide weekly individual therapy to members and complete required Department of Youth Service (DYS) clinical progress notes.

· Provide group therapy several times a week.

· Provide family therapy as needed.

· Develop an individual service plan for each member and create monthly treatment plan reviews with individualized goals and progress reports.

· Schedule and facilitate member treatment meetings (monthly) to review member progress. Act as a liaison as needed between youth, family, and DYS.

· Assess and monitor all clients on suicide watch in conjunction with the Clinical Director.

· Research and develop connections to community resources: including, but not limited to, employment opportunities and workforce training agencies in the community, educational programs, volunteer centers, and recreational activities.

· Support Youth Care Advocates (Youth Care Advocates) as mentors to the members of STRIVE.

· Provide training opportunities for YCAs in the areas of mental health, life skills, and asset coaching.

· Ensure that all program records and member files are accurate and up-to-date, including assisting with audit preparation, weekly filing, requesting records from DYS.

· Attend all trainings as scheduled.

· Report any suspected child abuse to the Clinical Director.

· Assist other Social Service Branch programs as directed.

· Assist with the volunteer and internship programs as necessary.

· Adhere to all fiscal policies regarding petty cash, vouchers, and purchase orders.

· Ensure DYS strategic plan is implemented as indicated.

FACILITY RESPONSIBILITY

Ensure the highest level of cleanliness on the Unit.

INTERDIVISIONAL/COMMUNITY/VOLUNTEER RESPONSIBILITIES

Actively participate in the Y’s Annual Strong Kids Campaign by completing all required tasks.

Offer support in crisis situations to all Old Colony Y programs.

EDUCATION/EXPERIENCE/PHYSICAL REQUIREMENTS

- Master’s Degree in Social Work, Counseling or a related field.

- Must have LCSW or LMHC, or be eligible for LMHC at time of hire.

- Maintain the security of program and clients.

- Work a flexible schedule.

- Must be able to walk, sit, stand during the work day.

Salary: $60,000-$73,000
