Job Description

SHARE (Supportive Housing Assistance to Reach Excellence) is a supporting housing service designed to assist persons with serious mental illness(es) in maintaining independent housing in the community. The case managers provide the support needed in each person's home environment and in the community. SHARE provides outreach and case management services to program participants. The clinician works with these individuals in the community to complete assessments and develop an Individualized Rehabilitation Plan (IRP). Additionally, the clinician provides clinical oversight for the case managers to support consumers.

Position Title: SHARE Program Clinician

Salary: $50,000-55,000

Responsibilities:

  • Perform therapeutic rehabilitative skill development
  • Maintain accurate documentation and client records
  • Recommend referral services as needed
  • Develop, implement and maintain client IRP
  • Provide education to client systems related to nutrition, symptom management, medication safety and management, and discharge planning
  • Provide transportation to clients as needed
  • Provide crisis intervention services as needed

Requirements

  • Graduation from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling
  • LAC/LSW required
  • LPC/LCSW preferred
  • One year of experience in the psychiatric/mental health field
  • Graduated from an accredited college or university in Social Work, rehabilitation counseling, psychology, criminal justice, counseling, or other related behavioral healthcare or counseling
  • Valid N.J. drivers license with less than 6 points

Since 1959, Bright Harbor Healthcare, formerly known as Ocean Mental Health Services, has been providing counseling to the residents of Ocean County. What was once a single office and two counselors has grown to an agency of over 300 staff, with four outpatient offices, a school, several partial day program locations and many outreach and community programs. Since our first expansion in 1961, into our office site on Lien Street, our growth has always been in response to the needs of the community.
