Job Description

Program Overview
Located in Columbus, OH, Sequel Pomegranate Health Systems provides treatment services to adolescents, ages 12 to 17, with mental health and behavioral disorders. We have a 20-bed acute hospital accredited by the Joint Commission for teens requiring intensive crisis care and treatment intervention.

We have an immediate opening for a licensed Therapist who will provide therapeutic services to patients in our
20-bed acute unit.

Position Duties
In collaboration with interdisciplinary teams, prepare treatment plans within the required admission timeframe.
Prepare individual service plans (ISP) in measurable and behavioral terms and ensure the ISP is updated accordingly.
Lead treatment team meetings and be prepared to lead the review of the adolescent’s overall progress on the individual service plan.
Ability to conduct individual, group and family therapy sessions, as needed, in a safe, therapeutic environment.
Update diagnostic assessments.
Attend treatment team meetings and review the patient’s overall progress.
Maintain contact with referring agency to update condition and progress of patient.
Assist with discharge planning to ensure a seamless transition.
Prepare reports and discharge summaries.
Prepare daily billable documentation.
Comply with applicable requirements under current licensure and within scope of practice.

Position Requirements
Master’s degree required. LSW/LPC license required, LISW/LPCC preferred.
Trauma informed care experience
CPR/First Aid Certification required.
Current Ohio social worker or counselor license.
Must have a minimum of two – four years’ experience in the behavioral health field, providing treatment to children and/or adolescents.
