Job Description

The Providence Center helps adults, adolescents and children affected by psychiatric illnesses, emotional problems and addictions by providing treatment and supportive services within a community setting.

The Providence Center Clinical Therapist Behavioral Health Outpatient (BHOP) Fee For Service will provide outpatient psychotherapy either at The Providence Center or in the community.

Duties and Responsibilities:

Perform a comprehensive psychosocial assessment of assigned consumers.

Complete crisis assessments, initial assessments and integrated summary forms.

Develop and implement a plan of care with ongoing consumer input.

Provide individual mental health and/or substance abuse treatment and crisis assessment.

Perform record keeping in accordance with Health Information Services and other Center requirements.

Other duties as assigned.

Requirements

Master’s degree in Social Work or related field required.

Candidate must have or be in the process of obtaining Independent license (LICSW and/or LMHC).

Must have a valid driver's license and insured auto.

Bilingual Spanish preferred.
