Responsible for weekly individual clinical supervision for case managers within the HIV Case Management Program, as well as completing continuing education requirements to increase competency in HIV care and with priority populations. The incumbent will also serve as a HIV Subject Matter Expert (SME) providing guidance on up-to-date best clinical practices for medical case management and non-medical case management for people with HIV. This role will require close collaboration with Equitas Health’s SME team to ensure compliance with the directives of the Human Resources and Services Administration (HRSA) and/or Ohio Department of Health (ODH). The individual will operate in accordance with the established professional standards and guidelines as stated by the Ohio Revised Code and put forth by the Ohio Counselor, Social Work and Marriage and Family Therapist Board as well as the National Association of Social Workers (NASW).

ESSENTIAL JOB FUNCTIONS:
Essential functions of the job include but are not limited to utilizing a computer for typing and attending meetings, travel and having reliable transportation to engage with the Ryan White funder and attend required meetings and working within health records.

MAJOR AREAS OF RESPONSIBILITIES:

• Provide a minimum of two (2) hours of individual high-quality supervision and one (1) hour of group supervision for each assigned Case Manager per month, with a focus on HIV viral suppression and associated illnesses and other improved HIV-related health outcomes through reviewing documentation, putting theory into practice, and advanced continuing education.
• Participate in grant sponsored trainings and meetings, including ODH/HCS-sponsored training for newly hired Part B case management personnel within 45-60 days of hire date, and case management supervisors’ trainings offered generally two times per year.
• Provide training and utilization support to staff related to HIV and HIV-related social determinants of health, ethical decision making, and documentation of services.
• Maintain a supervision contract and supervision notes with each clinical supervisee. Reviewing and signing supervision tracking logs monthly per funder and agency guidelines.
• Assure completion of reports and data collection for HRSA, ODH, and agency purposes.
• Coordinate with respective staff members on budgetary and fiscal operations to monitor expenditure of funding.
• Collaborate with site-based manager to evaluate staff caseloads through ongoing review of assessments, individual case plans, and other related documentation to ensure appropriateness for services, and ongoing engagement of client and client’s personal and formal supports.
• Conduct individual-level and group-level trainings with relevant Programs staff
• Provide assistance and coaching when lethality assessments and crisis intervention(s) are required. Coordinate with site-based manager as needed.
• Oversight of confidentiality practices to ensure all staff are adhering to confidentiality policies, HIPAA, and other established professional standards and guidelines.
• Coordinate with internal and external partners to assure a better integrated healthcare journey for people with HIV.
• Promotes a collaborative, cooperative, and productive working environment, as well as demonstrates sensitivity, team building, support, and respect.
• Representing the agency and supporting the HIV Community at local planning advisory groups, as assigned.
• Demonstrates continuous growth and development related to Equitas Health Values, exhibiting an understanding, awareness, and respect for diversity, inclusion, and cultural humility.
• Oversee onboarding and coordination of relevant Case Manager trainings in coordination with SME Department.
• Utilize email, Microsoft Office, phone, and other telecommunication options to participate in meetings across sites.
• Other duties as assigned related to the RWB grant and this position by supervisor.

EDUCATION/LICENSURE:

• Possess a master’s degree in Social Work
• Maintain active Licensed Independent Social Worker with Supervision designation (LISW-S) licensure within the state of Ohio.

Knowledge, Skills, Abilities and other Qualifications

• Have at least three years of experience as a social worker, preferably one year of supervisory experience.
• Must have sensitivity to, interest in and competence in culturally responsive care, HIV, minority health, sexual practices, substance use, chronic disease management, and a demonstrated competence in working with Black-Indigenous-People of Color (BIPOC), sexual minorities, and gender minorities. Ability to work harmoniously with diverse groups of individuals.
• A background in these HIV Subject Matter areas is preferred:
  • HIV Ryan White Care Act programming
  • HRSA-funded programming
  • Intersectional social work response across sexual & reproductive health, substance use, and societal trauma
  • Trauma informed care
  • Grant administrative support
• Proficiency in all Microsoft Office applications and other computer applications required.
• Must have knowledge and adherence to social work standards and ethics.
• Must have reliable transportation to be able to attend in-person trainings and meetings as needed, with advance notice.
